Our mission

Solving the foundation, not the surface.

To keep high-quality healthcare sustainable, with fewer nurses, more chronic disease, and limited hospital capacity, the sector needs to move faster. Today, progress is slow for structural reasons: reimbursement frameworks lag, regulation is heavy, and real organisational change is required.

TMP starts at the foundation. We make sure systems can speak with each other by solving data interoperability, enabling compliant transmural data providers to connect with hospital caregivers and GP practices so care pathways can evolve and outcomes can be integrated into clinical workflows.

Without a shared standard, every hospital-provider pair requires a bespoke project. With TMP, integrate once and reach the entire network. As more hospitals, practices, and providers join, integration becomes easier for everyone.

2021

The beginning: Telemonitoring Prescription

TMP originated at BeMedTech with the support of Agoria. Under the name Telemonitoring Prescription, the platform introduced a standardised request and return data flow between hospital EHR systems and external transmural data providers, marking the first step toward a shared interoperability standard for transmural care in Belgium.

2022 – 2023

Growing the network

The platform expanded beyond the initial pilot, connecting more hospitals and transmural data providers. Early production-ready EHR integrations were established and the foundation was laid for a broader, nationally supported rollout.

2024 – 2025

Telemonitoring Prescription: national project with FPS Health Belgium

Starting September 2024, TMP became the backbone of an officially supported national project, guided by OLV Waregem and backed by FPS Health Belgium, Agoria, and BeMedTech. The project set out to enable structured telemonitoring across Belgian hospitals at scale. 56 hospitals joined the consortium 35 of them actively monitoring patients in production. By the time the project closed on 31 December 2025, more than 6,500 patients had been monitored through the platform, far exceeding the original target of 1,000.

TMP iVZW founded

The governance structure was formalised with the founding of TMP iVZW. With caregivers in control, TMP iVZW was established as the mission steward, safeguarding neutrality and affordability so the platform remains a trusted, shared infrastructure for the entire healthcare ecosystem.

2026

TMP BV founded: pay-per-use launches

TMP BV was founded to run day-to-day operations: product delivery, implementation, partner onboarding, and service continuity. Together with TMP iVZW, the dual-entity structure is now complete. Contributing hospitals ensured TMP could continue its mission and introduced the pay-per-use model across all hospitals and practices. Organisations pay only when a patient session creates real integration value. The network now covers 15+ connected providers and 8+ EHR systems in active partnership.
