To keep high-quality healthcare sustainable - with fewer nurses, more chronic disease, and limited hospital capacity - the sector needs to move faster. Today, progress is slow for structural reasons: reimbursement frameworks lag, regulation is heavy, and real organisational change is required.

TMP starts at the foundation. We make sure systems can speak with each other by solving data interoperability - enabling compliant Transmural Data Providers to connect with hospital caregivers so care pathways can evolve and outcomes can be integrated into clinical workflows.

The interface for hospitals and healthcare professionals. Enables provider selection, patient context sharing, and operational transparency - fully integrated into the EHR with Single Sign-On.

The gateway for structured result delivery. Returns data from Transmural Data Providers to hospital EHRs in a consistent, HL7-aligned format. FAIR and FHIR compatible.

The backoffice for hospital management. Gives teams the tools to activate data providers for their hospital, manage data flows, inspect the data sent to providers, and monitor usage and governance insights.

From Belgium to Europe

TMP originated at BeMedTech with the support of Agoria. Originally called Telemonitoring Prescription, it introduced a standardised request and return data flow between hospital EHR systems and external providers in 2021.

Since 2024, the platform has scaled further with support from FPS Health Belgium and the eHealth Platform, with the ambition to extend the interoperability model across Europe.

In 2026, these contributing hospitals made sure TMP could continue its mission and introduce the pay-per-use model towards all hospitals.
